Before booking your event, she needs a little information regarding you and your event. Please fill out the form below and Pat will get back to your shortly.

After Pat has received your information and an agreement has been made on time, place, and the rate for her services, you will need to fill out this Performance Contract to supply Pat with the additional particulars of your event, such as music requested, attire she wears, etc. The form is a fill-able PDF, so you can fill it out on your PC, or you may print it out and write on it. Be sure to read it completely before sending it to her.

A deposit is also required to reserve those services at your event.
